2) You would need an enroller - i.e an introducer to proceed. In your downloaded form please use the 4Life id# 6784120 as the enroller/introducer.

3) After filling in your contact and credit card details, please scan/fax it back to us (+603- 77292328) or fax directly to the local 4Life office listed on the form
