4) Fill in your Bank details - this is for the company to deposit commissions or rebates (if any) to you. This bank account holder name must match the distributor's name.
Isi butiran akaun bank anda - deposit kommisen atau rabat (jika ada) kepada anda. Nama pemegang akaun bank harus lah bersamaan dengan nama pemohon.

5) Sign the form.
