The company requires hard copies to complete the sign up process. Here are the guidlines, the download links, scan and email to us.

Guide on completing the Distributor Application Form

1) Fill in your name, address, contact details,etc

2) Enroller information: Fill in as > : Yeap Eit Keung, 4Life id# 5100188

3) Sponsor Information: Fill in as> Yeap Eit Keung, 4Life id# 5100188

4) Sign the form.

Guide on completing the Order Form (to place your order of products)

1) Fill in your name etc, and where you wish the products to be shipped to.

2) Fill in the product you wish to buy now. (Don't worry about the "item number" )

3) Fill in your credit card details.

We encourage you to be on autoship because its many benefits - so consider to be on this. Autoship is like a subscription, - like you subscribe to a favorite magazine, or subscribe to cable TV etc.

That's all there is.

Fax or Email Details

Fax the completed forms to the 4Life office.
